What should I do if I forgot my password?1. Visit the Login 2. Click I forgot my password 3. Enter your email address. 4. Click Reset Password 5. An email will be sent to your address with instructions to follow. 6. Open the link provided in the email. 7. Confirm your new password. 8. Click Update Great! How do I arrange a demo?A demo is a 30 to 45 minute presentation in which our team provides an overview of the tools Sociants provides. Coordination of a demonstration: Click on the following link Enter your email address in the space provided. Click on Start booking. Select the date and time of the demo. Provide your full name, the name of your organization, the social determinants of health your organization works on, and the cities and regions it serves. Click Confirm. You will automatically receive an email with the details of your appointment. Additionally, one of our representatives will contact you shortly to confirm your demo.

How do I receive notifications from Sociants to my phone?When you use Sociants, you will receive push notifications via email and text messages to your mobile phone number. Additionally, you will be able to identify new alerts and notifications in the upper right corner of the Sociants portal. To set up your email or phone number, visit the My Account section and click the Modify Profile button. When you're done, click Update.

Is the data in my files safe at Sociants?At Sociants, our priority is to protect the information of each user and beneficiary of our platform. To achieve this, we implement protections to control who has access to the data and for what purposes. Our legal team is responsible for establishing and managing the legal structures and agreements that ensure that our platform complies with all state and federal regulations related to information sharing. It is our duty and commitment to protect your information and promote the maintenance of the highest degree of confidentiality, privacy and security, in accordance with the standards of the "Health Insurance Portability and Accountability Act" (HIPAA). Using Sociants is free?Registering with Sociants and using multiple digital tools for your organization is completely free. Our basic subscription for Community Providers includes: • Two (2) free users. • Access to the Community Provider Directory. • Ability to receive and attend to referrals. • Unlimited digital surveys. • Support resources and user guides. For information about our more advanced subscriptions, please contact info@sociants.com.

What do I do if I need more specific technical support?For any questions or problems when setting up your organization's account, you can use our in-platform chat service or send an email to info@sociants.com. One of our representatives will contact you within the next 24 hours.

In what languages is the platform available?Sociants can be used in Spanish and English. When you register, you will be able to select your preferred language in the My Account section. You can also change the language by pressing the green button in the upper right corner of the page. If you speak a language other than Spanish or English, please let us know by writing an email to info@sociants.com

Who uses Sociants?Sociants is used by different types of entities that are part of the social assistance system: Community Providers: entities or organizations that have the necessary resources to offer assistance in the community. The health sector: which has the capacity and resources necessary to identify and address the social determinants that affect the health of its beneficiaries. Government agencies: that provide assistance to citizens by offering essential services and community support. Social work students: who participate in the Sociants University program and use innovative technologies to improve the effectiveness of professional social work practice.

What is the vision of Sociants?At Sociants, our focus is on meeting people's social needs. Our vision is of a world in which the challenges posed by the social determinants of health can be overcome, with the goal of improving the well-being of people regardless of their circumstances.

What are Social Determinants of Health?According to the World Health Organization (WHO), social determinants of health (SDoHs) are the personal, social, economic and environmental conditions in which people are born, grow, live, work and age. These determinants are influenced by the unequal distribution of power, money and resources at the local, national and international levels. Therefore, they have a significant impact on people's access to healthcare and their overall quality of life. Sociants has adopted 13 social determinants of health, inspired by the conceptual framework developed by the Centers for Medicare and Medicaid Services (CMS): Medical care Security Substance use Housing Finances Community support Education Food Employment Disability Public services (water, electricity, etc.) Mental health Transportation By addressing these social determinants, Sociants aims to improve people's well-being and health outcomes, ensuring they receive the support and resources necessary to lead healthier, more fulfilling lives.

How does Sociants help Social Workers?At Sociants, we are 100% committed to strengthening Puerto Rico's social assistance system at multiple levels. For this reason, with our technology we support organizations and entities that provide direct services to people who come to them for help. We support social workers by providing Community Providers and their social workers with the digital tools necessary to facilitate the process of coordinating referrals and social support services. In this way, they can meet the needs of the population they serve quickly, accurately and safely.

Does Sociants provide services directly to individuals?Sociants does not provide direct services to individuals. Instead, Sociants offers Community Providers the digital tools necessary to streamline the coordination of social assistance services aimed at individuals and communities.
